All new patients will be asked to complete a short questionnaire if you are unable to complete you will be asked make an appointment for a registration medical. This will consist of an appointment with the practice nurse who will record your height, weight, blood pressure and test your urine. Failure to attend for a registration medical if required may jeopardise one's entitlement to register at this practice.
