Kuro's Charm and the option to give it away to increase the game's difficulty becomes unlocked as soon as the player has completed the main story and entered New Game Plus at least once. Parting with the charm will give you a permanent 'difficulty' debuff, similar to ringing the Demon Bell. This debuff makes you receive chip damage when blocking attacks, effectively changing your play style.
