The story of Sekiro: Shadows Die Twice is all about shades of gray, but one of the few definitively dark characters is Doujun.

He is a secretive, no-nonsense physician in the service of a mysterious man named Dosaku, who has tasked Doujun with delivering him a tough, strong warrior for some nefarious purpose. Having both Suppress Presence learned along with a Gachiin's Sugar in your inventory can make initiating the fight easier, as both Doujun and whomever he brings along can be Deathblowed from the back before becoming aware of you. Swim from the idol and climb up the wall, where you can use the Sugar, then go out and back-stab the one closest to you first. Both enemies are also susceptible to the Puppeteer Ninjutsu ; though it is recommended you use it on Doujun's experimental "friend" first, as it will automatically kill them afterwards, and Doujun is the lesser threat of the two. As "Red Eye" enemies, they are weak to fire and will both take increased Posture damage and get the Burn status fairly easily when attacked with it. Opening Suzaku's Lotus Umbrella , using the Flame Vent , Leaping Flame , or throwing down some Shinobi Firecrackers during the fight will also have the added bonus of making both of them cower back for a few seconds, allowing you some easy hits. Additionally, if you choose to send Kotaro over Jinzaemon, this fight can be trivialized by using the Divine Abduction prosthetic on Kotaro, as it will kill him instantly.

I'm at the part where you have to fight the red-eyed surgeon and the other dude I gave to him and I'm finding it impossible. There's no way to dispatch one with stealth and the area is the size of a closet. The closest I've come to winning is by jumping in the water, going around, and getting a pot-shot off whoever decides to charge me, but it took forever and they whittled me down. Doesn't help that they hit like a truck. Does anyone have a strategy for this? I use my flame vent but I run out of emblems before I even get their health to half. The surgeon explains that Hanbei 's superior type of immortality derives from the centipede in his body: this insect was imbibed in and adapted to the Rejuvenating Waters, giving him the power to regenerate the flesh of his host, no matter how severe his wounds are.

In Ashina Castle Hanbei and Shoukichi find themselves face to face with Takamine , who admits he sent those "brigands" to kidnap villagers for Doujun's experiments, under the orders of Genichiro. The samurai is victorious at last, but his friend was gravely injured during the fight and Doujun offers to save him, if Hanbei is willing to follow him into the Abandoned Dungeon. As the doctor is tending to Shoukichi , Hanbei is confronted yet again by Kodou Takamine , who Doujun healed from the Undying 's fatal blow with his Rejuvenating Sediment.

I'm "filled a with rage" that this error hasn't been noticed or fixed yet.

The way you fight them one on one is by getting Doujun to have Dragonrot before disappearing. I sent jinzaemon there and killed him before making doujun move there too.
