The Zentorno is heavily based on the Lamborghini Sesto Elemento , featuring the overall shape, hexagonal vents in the back of the car, twin roof scoops and triangular vents on the hood. The car's default color scheme when spawned at the player's garage in singleplayer is also reminiscent of the distinctive carbon-fibre-and-red colors of the Sesto Elemento. The front fascia, greenhouse area, rear diffuser, side vents and grille resemble that of the Lamborghini Veneno. Additionally, the car can be customized with a unique spoiler highly reminiscent of that on the Lamborghini Veneno. The rear fascia is largely based on the Sesto Elemento, while also taking elements from the Veneno. The sides of the car and the tail lights are derived from the Lamborghini Aventador. The Zentorno is designed as an exotic high-performance vehicle, with very aggressive styling cues and a low, sloped body for added aerodynamic effects. The front end is largely dominated by the numerous intakes, separated by carbon-fibre frames. The bonnet area features four small intakes on the centre and two triangle-shaped details over a carbon-fibre portion near the windscreen. The frontal area features two headlights located in black housings, as well as the manufacturer's emblem located between the lights. The sides of the car denote its aerodynamic profile, featuring concave details above the skirts and inset portions on the doors, together with vertical intakes on the rear fenders and diagonal body lines. Like the Infernus , the Zentorno features scissor doors with rounded mirror shells near the cabin A-pillars. However, these doors do not rotate away from the vehicle body, giving it realistic mechanics. The greenhouse area features coloured front separations behind the A-pillars and carbon-fibre panels on the rear quarters, while the area behind the cabin has two roof intakes leading to the engine bay, two rows of six hexagonal vents on the centre, small vents on the sides and two more vents over the rear tail lamps, finished in carbon fibre. The rear of the car is predominated by carbon-fibre elements, being the bumper, diffuser, and the separations on the rear vents.

Unlike its primary real-life inspirations, the Zentorno uses a single hexagonal exhaust pipe mounted at the bottom. The headlights are derived from the Acura NSX. The Zentorno also uses scissor doors , the second car in the entire series to do so, following the Infernus. The car actually is a hybrid supercar, giving it additional power and preventing power loss between gear shifts. Like with most of its real-life counterparts, the Zentorno is a rear mid-engine, all-wheel-drive supercar, balancing grip between the front and rear wheels, as well as giving it very good cornering ability. This also makes it unlikely to suffer heavily from front-end collisions compared with front-engined or rear-engined cars like the Surano and Comet.
